ABSTRACT

Objective:To establish a standardized information management system (IMS) for preserving, managing, querying, and performing statistics on biospecimens and their clinical data, which is conducive to improving the utilization of biobank.Methods:Under the premise of ensuring operating environment and data security, a database-based data logic relationship model is created and applied to the IMS to manage and analyze biospecimens and their supporting clinical information of patients enrolled in the biobank of our center.Results:To ensure the establishment of the follow-up cohort, biospecimens and clinical information of inpatients and outpatients were continuously collected in the biobank of our center. Since December 2014, more than 270 000 biospecimens from inpatient, outpatient, and scientific research have been preserved. The IMS optimized by this model efficiently completes the basic work of the biobank. At the same time, the data can be queried jointly and in batches, and then converted into a report format for statistical analysis.Conclusions:The IMS of our center is suitable for application and popularization as a construction and management model for the hospital-level biobank, which meets the daily work of the biobank and diverse research needs, and provides a convenient platform and rich resources for the development of precision medicine.

ABSTRACT

Objective@#To provide data support for AIDS prevention and control by investigating HIV testing among technician school students in Guangzhou.@*Methods@#A total of 1 112 students were investigated with a questionnaire about AIDS knowledge, attitude, sexual behavior and HIV testing through a stratified cluster random sampling method in October 2018. A Logistic regression analysis was conducted for influencing factors of students’ HIV testing.@*Results@#The HIV testing rate of technician school student was 10.3%. The HIV testing rate (12.1%-24.3%) of male, under 18 years of age, with monthly living expenses over 3 000 yuan and with poor family atmosphere was higher than that of the corresponding group (6.6%-8.4%), the difference were statistically significant (χ2=12.07,7.25,10.73,9.77, P<0.05). Multivariate Logistic regression analysis showed that <18 years of age, willingness to participate in relevant AIDS prevention activities, homosexuality, “support for multiple sexual partners” and having sexual behavior were associated with more HIV testing(P<0.05).@*Conclusion@#The incidence of sexual behavior of students in technician school is high while the rate of HIV testing is low, especially for male and male homosexuals.The health and education departments should strengthen students’ AIDS counseling and testing services and raise students’ awareness of AIDS risk in order to improve the coverage of students’ HIV testing.

Objective@#To investigate the effects of deer play of Wu Qin Xi on cervical mobility, TCM symptoms and anxiety in elderly patients with cervical spondylotic radiculopathy.@*Methods@#Sixty patients with orthopaedic inpatients from June 2017 to June 2018 were enrolled. In the 6 to December 2017, 30 inpatients who met the criteria were selected as the control group, and 30 patients who met the criteria from January to June 20 were the intervention group. Two groups of patients were treated with conventional nursing, and the intervention group was treated with deer play on the basis of the control group. Cervical dysfunction index (NDI) was compared before intervention, 2 weeks and 4 weeks after intervention; clinical symptoms and signs and neck activity were compared before intervention and 2 weeks after intervention; anxiety scores were compared before intervention and after 4 weeks of intervention (GAD-7).@*Results@#The NDI scores of the intervention group were 19.23±2.19 and 9.23±1.85 at 2 and 4 weeks respectively. The scores of the control group were 21.37±1.40 and 11.63±2.95, respectively. The difference was statistically significant (t=3.77-4.49, P<0.05). After 2 weeks of intervention, the clinical symptoms and signs and neck activity of the intervention group were 5.47±1.78, 19.17±1.26, 4.87±0.78, 11.50±1.46, the control group were 7.40±2.53, 16.23±1.87, 6.17±1.26, 13.17±1.56, and the difference between the two groups was statistically significant (t=-7.12-4.81, P<0.05). After 4 weeks of intervention, the GAD-7 score of the intervention group was 1.07±0.20, and the control group was 1.56±0.29. The difference between the two groups was statistically significant (t=6.55, P<0.05).@*Conclusion@#Luxi can improve the symptoms of neck pain in elderly patients with cervical spondylotic radiculopathy, improve cervical function and relieve anxiety.

ABSTRACT

An in vitro blood-brain barrier (BBB) model is critical for enabling rapid screening of the BBB permeability of the drugs targeting on the central nervous system. Though many models have been developed, their reproducibility and renewability remain a challenge. Furthermore, drug transport data from many of the models do not correlate well with the data for in vivo BBB drug transport. Induced-pluripotent stem cell (iPSC) technology provides reproducible cell resources for in vitro BBB modeling. Here, we generated a human in vitro BBB model by differentiating the human iPSC (hiPSC) line GM25256 into brain endothelial-type cells. The model displayed BBB characteristics including tight junction proteins (ZO-1, claudin-5, and occludin) and endothelial markers (von Willebrand factor and Ulex), as well as high trans-endothelial electrical resistance (TEER) (1560 Ω.cm ± 230 Ω.cm) and γ-GTPase activity. Co-culture with primary rat astrocytes significantly increased the TEER of the model (2970 Ω.cm to 4185 Ω.cm). RNAseq analysis confirmed the expression of key BBB-related genes in the hiPSC-derived endothelial cells in comparison with primary human brain microvascular endothelial cells, including P-glycoprotein (Pgp) and breast cancer resistant protein (BCRP). Drug transport assays for nine CNS compounds showed that the permeability of non-Pgp/BCRP and Pgp/BCRP substrates across the model was strongly correlated with rodent in situ brain perfusion data for these compounds (R = 0.982 and R = 0.9973, respectively), demonstrating the functionality of the drug transporters in the model. Thus, this model may be used to rapidly screen CNS compounds, to predict the in vivo BBB permeability of these compounds and to study the biology of the BBB.

ABSTRACT

Objective To determine the relationship among structural empowerment, occupational benefit and job embeddedness of nurses, and to analysis the role of occupational benefit in structural empowerment and job embeddedness. Methods A total of 422 nurses recruited from five tertiary hospitals in Tianjin. They were investigated with General Information Questionnaire, Condition of the Work Effectiveness Condition Questionnaire, Nurses' Occupational Benefit Scale and the Work Embedded Scale. Results The mean scores of nurses' work efficiency condition questionnaire, occupational benefit, and job embeddedness were (3.14 ± 0.88), (4.27 ± 0.53), (3.64 ± 0.677) points respectively. Structural empowerment, occupational benefit and job embeddedness were positively correlated (r=0.646, 0.536, 0.521, P<0.01);The nurses structural empowerment had significant prediction effect on work engagement (β=0.646, P<0.01),occupational benefit was a partial intermediary between structural empowerment and job embedding. Conclusion Through reasonable authorization, nursing managers actively guide nurses' sense of benefit, improve the close integration of nurses and organizations, then improve the level of nurses' work embedding.

ABSTRACT

Objective To understand the current situation of pediatric nurses' perceived stress,coping style and professional burnout,analyze the influence of pediatric nurses' perceived stress and coping styles on professional burnout,and to provide references for reducing pediatric nurses' professional burnout.Methods A questionnaire survey was conducted on 415 pediatric nurses from 9 tertiary level A hospitals in Tianjin using general information questionnaires,Chinese Perceived Stress Scale(CPSS),Simplified Coping Style Questionnaire (SCSQ),and Maslach Burnout Inventory(MBI).Results Pediatric nurses'MBI scores were divided into emotional exhaustion score (25.19 ± 9.98) points,depersonalization total score (10.70 ± 4.65) points,personal achievement sense (29.72 ± 9.62) points,and higher degree of burnout.Correlation analysis showed that the perceived stress was positively correlated with emotional exhaustion and depersonalization dimensions,and negatively correlated with the dimension of personal accomplishment (r =0.398,0.236,-0.388,P <0.01);positive coping dimensions were negatively correlated with emotional exhaustion and depersonalization dimensions,and positively correlated with the dimension of personal accomplishment(r =-0.544,-0.205,0.608,P<0.01);negative coping dimensions and emotional depletion and depersonalization dimensions were positively correlated,and negatively correlated with individual achievement dimension (r =0.559,0.298,-0.652,P <0.01).Multiple stepwise regression analysis showed that perceived stress and negative coping style had influence on emotional exhaustion,depersonalization,and individual achievement dimension.Positive coping style had influence on emotional exhaustion and personal achievement dimension (R2=0.105-0.501,P<0.01).Conclusions Pediatric nurses have higher professional burnout.Perceived stress and coping styles have an impact on professional burnout.Pediatric nurses should take more active measures to relieve perceived stress and reduce burnout.

ABSTRACT

Objective To investigate the clinical therapeutic effect of Xianyimucao capsule on hormone level and menstrual recovery in patients with vaginal bleeding after drug abortion.Methods 102 pregnant women with drug abortion admitted in our hospital from June 2015 to December 2016 were selected and divided into two groups by random number table, the control group was given conventional hemostasis and such symptomatic treatment as anti-infection, the observation group was given Xianyimucao capsule on the basis of control group.The clinical efficacy, the levels of serum hormones and menstrual recovery before and after the treatment of the two groups were compared.Then the safety of two groups were evaluated.Results After 14 days of the treatment, the total effective rate of the observation group was 96.1%, which was significantly higher than 80.4% of the control group (P<0.05).And the serum HCG, P and E2 levels in the two groups after the treatment were significantly lower than those before the treatment (P<0.05);serum hormone levels of the observation group were significantly better than that the control group (P<0.05).Compared with the control group, the menstruation recovery of the observation group was significantly shortened (P<0.05);there was no significant difference of the first menstrual cycle and menstrual volume after the menstruation recovery between the two groups.There was no obvious adverse reaction in the two groups during meditation. Conclusion The application of fresh motherwort capsules in the treatment of vaginal bleeding after drug abortion can stop bleeding rapidly , regulate the body's hormone levels and recover normal menstruation with high safety .

ABSTRACT

Hypoxia is frequently observed in solid tumors and also one of the major obstacles for effective cancer therapies. Cancer cells take advantage of their ability to adapt hypoxia to initiate a special transcriptional program that renders them more aggressive biological behaviors. Hypoxia-inducible factors (HIFs) are the key factors that control hypoxia-inducible pathways by regulating the expression of a vast array of genes involved in cancer progression and treatment resistance. HIFs, mainly HIF-1 and -2, have become potential targets for developing novel cancer therapeutics. This article reviews the updated information in tumor HIF pathways, particularly recent advances in the development of HIF inhibitors. These inhibitors interfere with mRNA expression, protein synthesis, protein degradation and dimerization, DNA binding and transcriptional activity of HIF-1 and -2, or both. Despite efforts in the past two decades, no agents directly inhibiting HIFs have been approved for treating cancer patients. By analyzing results of the published reports, we put the perspectives at the end of the article. The therapeutic efficacy of HIF inhibitors may be improved if more efforts are devoted on developing agents that are able to simultaneously target HIF-1 and -2, increasing the penetrating capacity of HIF inhibitors, and selecting suitable patient subpopulations for clinical trials.

ObjectiveTo investigate the clinical guiding significance of Guideline for diagnosis and treatment of herb-induced liver injury in the diagnosis of herb-induced liver injury (HILI). MethodsA retrospective analysis was performed for the clinical data of 595 hospitalized patients who were admitted to 302 Hospital of PLA from January 2009 to January 2014 and diagnosed with HILI according to HILI diagnostic strategies recommended by American College of Gastroenterology. The chi-square test was used for comparison of categorical data between groups. ResultsAccording to the Guideline for diagnosis and treatment of HILI, all 595 patients (100%) were diagnosed with suspected HILI, 234 (39.3%) were diagnosed with clinical HILI, and 52(8.7%) were diagnosed with definite HILI. Among the patients with clinical and definite HILI, 85.0% and 94.2%, respectively, had a clinical type of cell injury, and 52.6% and 423%, respectively, had a severe disease. There were no significant differences in prognosis between the patients with suspected, clinical, and definite HILI, and 81.5%, 82.5%, and 80.8%, respectively, were cured. ConclusionGuideline for diagnosis and treatment of HILI can effectively guide the diagnosis of HILI, avoid misdiagnosis in clinical practice, and guarantee the safe application of Chinese herbal medicine. Its clinical guiding significance in the diagnosis of HILI awaits more prospective studies.

A random sample survey was conducted in 17 community medical institutes in 4 districts in Beijing.Among an effective sample of 385 residents.47.5% (n =183)of community residents chose community medical institutes for first contact care while 52.5% (n =202) opted for class other hospitals.Residents with lower education levels tended to choose community medical institutes (P =0.01).And those with chronic diseases preferred to choose communities (P=0.00).

ABSTRACT

Nonspecific inflammatory response is the major cause for failure of islet grafts at the early phase of intraportal islet transplantation (IPIT). Bilirubin, a natural product of heme catabolism, has displayed anti-oxidative and anti-inflammatory activities. The present study has demonstrated that bilirubin protected islet grafts by inhibiting nonspecific inflammatory response in a syngeneic rat model of IPIT. The inflammation-induced cell injury was mimicked by exposing cultured rat insulinoma INS-1 cells to cytokines (IL-1beta, TNF-alpha and IFN-gamma) in in vitro assays. At appropriate lower concentrations, bilirubin significantly attenuated the reduced cell viability and enhanced cell apoptosis induced by cytokines, and protected the insulin secretory function of INS-1 cells. Diabetic inbred male Lewis rats induced by streptozotocin underwent IPIT at different islet equivalents (IEQs) (optimal dose of 1000, and suboptimal doses of 750 or 500), and bilirubin was administered to the recipients every 12 h, starting from one day before transplantation until 5 days after transplantation. Administration of bilirubin improved glucose control and enhanced glucose tolerance in diabetic recipients, and reduced the serum levels of inflammatory mediators including IL-1beta, TNF-alpha, soluble intercellular adhesion molecule 1, monocyte chemoattractant protein-1 and NO, and inhibited the infiltration of Kupffer cells into the islet grafts, and restored insulin-producing ability of transplanted islets.

ABSTRACT

objectlve To investigate the efficacy of continuous low-dose fluorouracil with cetuximab for antiangiogenic effect on colon carcinoma xenograft,and test its antitumor effect and toxicity.Methods Balb/c mice bearing CT-26 colon carcinoma xenograft were randomly divided into five groups,receiving low-dose metronomic(LDM)fluorouracil,maximum tolerated dose(MTD)fluorouracil,cetuximab,LDM fluorouracil with cetuximab therapy and saline respectively.Tumor growth,weight loss,peripheral white blood cell counts and survival of mice were monitoted.At the end of experiment,tumors were resected for tumor microvascular density(MVD)by immunofluorescence staining. Results Tumor growth inhibition was found in mice receiving LDM fluorouracil therapy and combined therapy,without significant body weight loss or leukopenia,and the survival of mice was remarkably prolonged,compared with mice receiving MTD fluorouracil or cetuximab therapy,and the antitumor effects of the combined therapy was stronger than that of the fluorouracil LDM therapy.LDM treatment and combine treatment led to statistically significant(P<0.05)55%and 71%reduction in tumor growth,as well as 73%and 77% reduction in tumor microvessel density compared with the control respectively.Additonally,tunnel staining shows no significant difference between these treatment groups. Conclusion Continuous low-dose regimen of fluorouracil with cetuximab can significantly increase the therapeutic activity with decreased toxicity and prolonged animal survival bearing implanted colon cancer.

ABSTRACT

Vasostatin, a 180-amino acid fragment from the N-terminal domain of calreticulin, is a potent endogenous angiogenesis inhibitor, which can inhibit the growth of many kinds of experimental tumor. But a recent report that vasostatin can enhance the malignant behavior of neuroendocrine tumor reminds us to be cautious to develop it as an anti-tumor medicine. VAS cDNA was cloned into pAAV-2 expression vector; recombinant virus rAAV-VAS was generated by a three plasmids, helper free packaging method. MS1 mouse pancreatic endothelial cell and human colon tumor HCT-116 cell were infected with rAAV-VAS. Transgene expression was analyzed by Western blotting analysis; cell proliferation was determined by MTT assay. The therapeutic potential of rAAV-VAS was evaluated in subcutaneous HCT-116 xenograft mouse model. rAAV-VAS inhibited the proliferation of MS1 but not HCT-116 cell. HCT-116 cell infected with rAAV-VAS secreted VAS protein into the supernatant effectively. The intratumoral delivery of rAAV-VAS inhibited the xenograft growth and microvessel density in tumors significantly. Our results show the effectiveness of rAAV-VAS as an angiogenesis inhibitor in suppressing tumor growth, validating the application of rAAV-VAS gene therapy in treatment against colon cancer.

ABSTRACT

OBJECTIVE To explore the management methods of sterilization and supply department for the control of hospital infection. METHODS Since 2005, strict infection management system in all aspects by the sterilization and supply department has been taken including renewable medical equipment from recycle to distribution, sterile disposable medical equipment from acceptance to distribution, and the district environment and self-protection of its own staff in the sterilization and supply department and so on. RESULTS Hospital infections had not occurred as a result of sterilized supply for three years. CONCLUSIONS Since strict control and management system is carrying out, measures are effective, and good results have been achieved, which provide important guarantees for effectively controlling infection and medical safety.

OBJECTIVE To analyze occupational harmful factors in department of disinfection and supply and take protective measures in pertinency.METHODS We analyzed every tache of work flow in the course of disinfection and supply,and found out the potential occupational harmful factors.Based on them,the easy and feasible protective measures were proposed.RESULTS There existed many occupational harmful factors in the course of disinfection and supply.Therefore,the working environment and occupational protection among staff should be improved.CONCLUSIONS Analysis on the peril of occupational harmful factors,enforcement of protective consciousness and establishment and implementation of operation rules and protective measures are important steps of occupational safety management for staff in department of disinfection and supply.

ABSTRACT

OBJECTIVE To investigate the procedures and advantages of whole range quality control of disposable sterile medical appliance. METHODS By analyzing the data concerning whole range quality control of disposable sterile medical appliance from Oct 2002 to Oct 2005 in our hospital to summarize the experiences in this field. RESULTS During the last 3 years, the supply room in our hospital had administrated disposable sterile medical appliance 30 069 000 articles altogether, with the qualification ratio of 100%, and no adverse reaction was found; furthermore, the waste was reduced for no massed articles occurred; the cost accounting was raised and the error ratio during delivery was reduced too, the annual error ratio dropped form 0.003% to 0.00066%. CONCLUSIONS The whole range quality control of disposable sterile medical appliance can raise the administration quality, reduce the waste during delivery, and guarantee the disposable sterile medical appliance being used safely and effectively, also, it can provide accurate and useful data of cost accounting to concerned departments.

METHODS The standards and methods set in the Hospital Disinfecting Sanitation Standard(GB15982-1995) were used. RESULTS Scientific,effective and strict control measures should be implemented to make the quantum of bacteria existing in the air to meet the Environment Standard Type Ⅱ. CONCLUSIONS This article provides the credible data for boosting the pureness of the air of the cleaning section of disinfecting supply division.

OBJECTIVE To analyze the cause of failure of bacteria-killing by autoclave.METHODS Under the process of using the fast-reading bio-indicator to take a routine inspect for the autoclave,while the pressure,the temperature and the time for the bacteria-killing were up to the standard,and the result of the inspection was respectively negative and positive.RESULTS For the sudden malfunction of the exhaust valve,the cold air mass was left in the experimental parcel.Therefore,the result was respectively negative and positive.CONCLUSIONS The more strict examination should be taken for the bacteria-killing tank and the advanced B-D process inquiry-device and the bacteria-killing process inquiry device in the world should be adopted to ensure that the process of bacteria-killing is qualified at all times.
